RELEASE AND AUTHORIZATION

In connection with my application for employment with Davidus Cigars, Ltd., I understand that investigative background inquiries are to be made on myself which may include consumer credit, criminal history, motor vehicle, education, business associations, prior employers and other reports as needed. These reports will include information as to my character, work habits, performance and experience along with reasons for termination of past employment from previous employers. Further, I understand that you will be requesting information from various Federal, State and other agencies which maintain records concerning my past activates relating to my driving, credit, criminal, civil and other experiences. I understand that if I accept employment with Davidus Cigars, Ltd. I am agreeing to uphold the stick laws restricting the sale of tobacco products and related tobacco products to minors. I understand that if I sell a tobacco products to a minor, I may be immediately terminated from employment with Davidus Cigars, Ltd. I also understand that Davidus Cigars, Ltd. has a strict policy that requires employees to work their scheduled assigned shifts. I understand that if I fail to work a scheduled shift, I may be immediately terminated from employment with Davidus Cigars, Ltd.

I authorize, without reservation, any party or agency contacted by Davidus Cigars, Ltd.and/or Business Research & Intelligence, Inc. to furnish the above mentioned information.

I hereby consent to Davidus Cigars, Ltd. obtaining the above information from Business Research & Intelligence, Inc. and any of their licensed agents. I hereby agree to adhere to Davidus Cigars work schedule and selling products to minors policies above-mentioned.
